Making An Appointment

To make an appointment with any of our therapists call our West Des Moines (515-222-1999) or Ames Offices (515-233-1122) and talk with a receptionist.

Click on the appropriate button below for the West Des Moines or Ames Office or for help with nursing homes. You will then be taken to a page with forms we will need to schedule an appointment.

Please bring your completed forms with you to your appointment or arrive at least a half hour early to complete these documents for your therapist.

Please note; in January 2011 our fees will be increasing so that we may continue to effectively serve you. Fee information can be found in our HIPPA Consent and Agreement Form or you may call our office for more information.
